Health Insurance |
Haven Haven Hospice provides health insurance coverage for all full-time and part-time employees through AvMed Health Plan. The premium for the HMO Plan for the full-time employee is covered 90% by Haven Haven Hospice and the premium for the CHOICE Plan for the full-time employee is covered 95% by Haven Hospice; the premiums for part-time employees are prorated based upon the number of regularly scheduled hours weekly. Haven Hospice pays 10% of the premium for dependent coverage and 25% of the premium for family coverage. |
Dental |
Haven Hospice provides dental coverage for all full-time employees and part-time employees on a 30-hour a week schedule. Twenty percent (20%) of the premium is covered by Haven Hospice for both full-time and part-time employees and their dependents. |
Life & AD&D Insurance |
Full-time employees are provided with company-paid life insurance. The policy is equal to one times current annual salary, rounded to the next $1,000. E mployees can purchase supplemental life insurance in increments of one, two, three and four times current annual salary.

Long-term Disability |
Full-time employees are eligible for company paid long-term disability insurance. The company-paid benefit is 60 % of base monthly salary and the employee has the option of purchasing additional long-term disability coverage. The options include either 66 2/3 coverage or 70% coverage. |
Short-term Disability |
Full-time employees have the option of purchasing short-term disability insurance to replace a portion of his or her salary in the event of total disability. Premiums are based on the employee's current salary. |
Child Life Insurance |
Additional life insurance can be purchased at group rates for dependent eligible children. |
Spouse Life Insurance |
Additional life insurance can be purchased at group rates for current spouse. |
Long-term Care |
Full-time employees are eligible to purchase Long Term Care insurance for themselves, their spouse, parents, grandparents, parents-in-law and grandparents-in-law at Haven Hospice group rates. |
401(K) Retirement Savings Plan |
All employees are eligible to participate in the 401 (k); after one year of employment Haven Hospice will match contributions at 100% for the first 3% and 50% of the next 2%. |
Employee Assistance Program |
Haven Hospice offers an Employee Assistance Program to all full-time and part-time employees. This is a confidential program that assists employees and their families with personal issues and concerns. Services include 24-hour intervention and up to 5 in-person assessment visits with a licensed mental health professional at no charge. |
Medical Flexible Spending Account/
Dependent Care Spending Account |
Full-time and part-time employees are eligible to participate in both the medical flexible spending account and dependent care flexible spending account. These programs allow employees to put pretax dollars into a special account to be reimbursed for eligible health care and dependent care expenses. |
Paid Time Off |
Haven Hospice offers paid time off to all full-time and part-time employees. Employees are eligible to take paid time off after successfully completing the 90-day Introductory Employment Period. PTO days accrue at the rate 5.539 hours pay period (18 days a year) during the first 35 months of employment. The accrual rate increases for each subsequent year of employment (up to ten years). Part-time employees accrue on a prorated basis. |
Paid Time Off Cash-in |
Up to 40 hours once a year during the 30-day period after anniversary date can be cashed in; 3 weeks or 120 hours must be left in PTO bank. Hours are cashed in at the rate of 75 cents on the dollar. |
Paid Time Off Rollover |
PTO rolls over annually up to the allowed maximum. |
Extended Illness Bank |
EIB accrues at 5 days per year and rolls over annually; maximum accrual 520 hours. Provides for paid time protection to employees in the event of extended illness or injury. |
Beeper Pay and On-Call Pay |
Beeper Pay: Employees may be eligible for beeper pay compensation, in accordance with their job position and responsibilities.
On-Call: Nurses and social workers and other staff with patient care responsibilities receive on-call pay for visits that begin after 5:00 p.m. and before 8:00 a.m. and that are both outside their caseload and requested and approved by the team manager. |
Holiday Pay |
Haven Hospice recognizes 6 holidays and 2 floating holidays. |
Tuition Reimbursement
Professional Growth & Development |
Haven Hospice offers tuition reimbursement and reimbursement for professional seminars; $750 a year for full-time employees and $375 a year for part-time employees. Courses and seminars must be approved in advance and directly related to the employee’s position at Haven Hospice. |
Mileage |
Employees who use their own vehicle for company business are eligible for mileage reimbursement. Our current reimbursement rate is $0.405 per mile. |
Wellness Program |
Full-time and part-time employees who join a fitness club of their choice may receive reimbursement for membership dues from Haven Hospice for consistent attendance. |
Milestone Awards |
Employees who complete five (5) year increments of continuous service with Haven Hospice will receive special recognition. |
Jury Duty |
Haven Hospice encourages employees to accept their civil responsibility when called upon for jury duty or as required as a witness. Full-time and part-time employees who make prior arrangements with their a manger or director will be paid for time spent on jury duty up to two (2) weeks per year. Pay is granted at their current regular rate and only for the days regularly scheduled to work. |
Bereavement Leave |
Full-time and part time employees are eligible for bereavement leave upon completion of 90-day introductory period and may take off up to five consecutive days with pay if an immediate family member should die. |
