  • Meeting with patients and families within two hours of your referral – often more quickly if the patient is in the hospital and we have collaborated to create a Haven Hospice Hospital Liaison program in your facility.
  • Providing your facility a Haven Hospice Hospital Liaison. This person is dedicated to helping you optimize care for terminally ill patients. They can help you dialogue with patients and families about end-of-life care choices, interact with the attending physician about the discharge care plan, facilitate transportation and home medical equipment, and interact with long-term care facilities or assistedliving communities about placement – in short, support your discharge planning efforts for terminally ill patients.
  • A Haven Hospice staff member (not an answering service) when you call us, any time of day or night. We are always ready to admit one of your patients to our service, if they meet hospice criteria.
We know how important it is that the healthcare organizations you partner with demonstrate clinical excellence and inspire confidence in your attending physicians. Our service promise includes:
  • Five full-time and six part-time medical directors, two full-time pharmacists and three full-time advanced registered nurse practitioners on staff. These palliative-care experts are available for you and your attending physicians to collaborate with – ensuring that your patients will receive the care they need to assure optimal pain and symptom management.
  • Organization-wide expertise in using peer-reviewed tools and Medicare, Medicaid and other payor-approved guidelines for prognosticating survival – consistent with the hospice eligibility criteria of the payors and the State of Florida’s Hospice Licensure Law.
  • The ability for your patients to be treated with state-of-the-art pharmaceuticals or biologics or other clinical interventions, when they are appropriate to palliate the patient’s pain and other symptoms; and Haven Hospice clinical staff who stay abreast of how to use these new treatments.
Excellence
  • Haven Hospice’s expertise, which extends to the complex psycho-social issues relating to “breaking bad news” to patients. We can directly assist with this process or provide training materials based upon peer-reviewed literature.
  • An array of CEU programs for your staff. Ask for our always-expanding full list of available programs.
  • Haven Hospice’s definition of clinical excellence, extending beyond “physical” care to emotional, psycho-social and spiritual care. Haven Hospice’s staff of social workers and chaplains has unrivaled experience and expertise in working with terminally ill patients and their families to help them at end of life. In addition, as a community agency, Haven Hospice offers programs in grief and loss for all community members who need them; only Haven Hospice provides this support for the community.

In north Florida, Haven Hospice has more contracts and relationships with long-term care facilities and assisted-living communities than other hospices – and Haven Hospice’s broad array of unique inpatient Care Center environments is simply unavailable elsewhere.
  • Caring for all patients, regardless of their ability to pay for their care. As a not-forprofit, community-based agency, we have never and will never turn eligible patients away – whatever their needs, whatever their care plan.

Our service promise includes:
  • Keeping you informed of our overall patient satisfaction scores
  • Conducting a regular review meeting (monthly or quarterly), in which you give us formal feedback about how we are doing in meeting your needs (for example, how we can continue to improve our working relationship or how our Haven Hospice representatives can improve their interaction with your staff)
