The Haven Transitions program is a community outreach effort that provides support, education, and community resource information to individuals and families dealing with potentially life-limiting illnesses.

The Haven Transitions program is a "phone contact" service designed to facilitate discussion of options for care, decision making, and the medical, emotional, legal, financial and spiritual needs of patients and family members. 

• It is not necessary to become or be a hospice patient to benefit from the Haven Transitions program.
• Supports individuals and their families when they become seriously ill.
• Provides support and listens to your challenges.
• Does not charge a fee or require paperwork for services.
