We know how important it is that the health-care organizations you partner with demonstrate clinical excellence and enable you to practice state-of-the-art medicine. Our service promise includes:
  • Six full-time and six part-time medical directors on our staff with whom you can collaborate. We recognize that there are times you need to interact with a physician, particularly one dedicated to staying abreast of clinical developments in palliative care, to assure the type of care you think appropriate for your patients.
  • Two full-time pharmacists on our staff with whom you can collaborate. Haven Hospice’s pharmacists are dedicated to staying up to date with developments in palliative pharmacology, providing you unique resources for assuring patient comfort.
  • Three advanced registered nurse practitioners on staff – helping to optimize patient care.
  • Organization-wide expertise in using peerreviewed tools and Medicare-approved guidelines for prognosticating survival – consistent with hospice eligibility criteria.
  • The ability for you to treat patients with state-ofthe- art pharmaceuticals or biologics or other clinical interventions, when they are appropriate to palliate the patient’s pain and other symptoms; and Haven Hospice clinical staff who stay abreast of how to use these new treatments.
  • Haven Hospice’s expertise, which extends to the complex psycho-social issues relating to “breaking bad news” to patients. We can directly assist with this process or provide training materials based upon peer-reviewed literature.
We know how important it is for you to work with healthcare organizations that are responsive and reliable – at all times of day or night. Our service promise includes:
  • Meeting with patients and families within two hours of your referral
  • The ability to contact a Haven Hospice staff member (not an answering service) for up-to-date information about your patient, any time of day or night
  • Available staff at all times for on-site care when a patient’s needs change, requiring in-person intervention
We know how important it is for you to have confidence that your partner can care for all your terminally ill patients and meet their changing needs. Our service promise includes:
  • Serving a wider geographic area than any other local end-of-life care provider, assuring that we can serve your patients in the area.
  • The unrivaled ability to help you care for your patients in whatever setting is most appropriate. In north Florida, Haven Hospice has more contracts and relationships with long term care facilities and assisted-living communities than other hospices – and the array of Haven Hospice’s unique inpatient Care Center environments is simply unavailable elsewhere.
  • Caring for all patients, regardless of their ability to pay for their care. As a not-for-profit, community-based agency, we have never and will never turn eligible patients away – whatever their needs, whatever their care plan.
  • If you wish, one of our physicians to be your eyes and ears in the patient’s home (whether their traditional residence or a long-term care facility or assisted-living community) or, if they are in the hospital, by making an on-site visit to the patient – helping you best evaluate the overall situation. This can even occur before a patient is enrolled as a hospice patient.
We know how important it is to communicate with you about your patients – consistent with how you practice, consistent with your needs. Our service promise includes:
  • Deferring to what role you choose to play with each of your patients, meaning whether you wish to remain the attending physician or collaborate with our physician acting as the attending. We work with you to customize what information you need (the list below is a sample, rather than all inclusive) and the way in which you want us to contact you (office phone, cell phone, fax, e-mail, etc.):
    • Admission confirmation
    • Change in level-of-care status
    • A monthly summary of patient care
    • Interaction regarding the evolving plan of care, medication changes,

  • equipment needs and staffing needs
    • Updates regarding a patient’s death or discharge
We know that you must have confidence in the ability of your partners to help you get appropriate reimbursement for your services.

Advanced Care

Our service promise includes:
  • A full-time physician billing liaison to help you with billing issues.
  • “How to” information that can be reviewed and left with your office staff.
  • Billing seminars a number of times each year, which can also be held for individual offices if appropriate.
