The Haven Hospice Referral Process

Although many referrals are made by physicians, friends, family members and clergy, anyone can refer a patient to Haven Hospice or simply call to inquire about the array of services Haven Hospice provides. At the time of referral Haven Hospice will follow up with the patient and family WITHIN TWO HOURS of the call. A Haven Hospice staff member – not an answering service – will answer the call anytime, day or night. Our team will then contact the patient's personal physician to explain services and work collaboratively to determine if the patient is ready for hospice care.

  • Physicians may refer patients to Haven Hospice who have a prognosis that exceeds six months. These patients may be referred to the Haven Transitions program for follow up phone contact.
  • When Haven Hospice receives an inquiry regarding services or requests for assistance with a patient that does not yet qualify for hospice care, our skilled professionals will provide information about community support that could be of service.

  • Haven Hospice provides services to patients who have life-limiting illnesses regardless of their ability to pay.
  • Haven Hospice has more contracts and relationships in north Florida with long-term care facilities, hospitals and assisted-living communities than any other hospice care provider.
  • Haven Hospice offers stand-alone care centers that provide a home-like environment for patients and loved ones when the need arises.
  • Haven Hospice patients that qualify for services under the Medicare and/or Medicaid hospice benefit must have a prognosis of six months or less if the disease follows its normal and expected course.
  • Haven Hospice provides services to children and young adults with life-limiting illnesses even if the prognosis is uncertain.
  • Haven Hospice care is focused on the management of symptoms to maximize the quality of life. The aim of care is comfort not curative.
  • Haven Hospice does not require a "Do not Resuscitate" (DNR) in order to provide care. Hospice staff members are trained to discuss resuscitation choices with the patient and/or the patient's designee as appropriate.
  • Haven Hospice does not require that the patient have a primary caregiver in the home. Hospice staff is trained to discuss and assist in planning should the patient reach a point when he or she can no longer care for themselves at home.
